Where families start

Most families reach us in the middle of one of these.

They’re coming home and nobody feels ready

An RN sets up the medications, the equipment and the first nights home. Before discharge, not after.

I’m the spouse, the driver and the nurse

We take the clinical half back, so you can go back to being their partner.

I live far away and I’m always guessing

The same RN calls you after every shift. No chasing, no second-hand updates.

We already have caregivers we trust

Keep them. We add the clinical judgment a caregiver isn’t licensed to give.

We don’t know what level of care we need

Start with a call. An RN will tell you plainly, even if the answer isn’t us.

Matched care

The certification matched to the diagnosis.

Our standard

By the numbers

One team

The same small care team every visit, so you never re-explain the history.

< 2 hrs

From your referral to a real conversation with the family.

24/7

Skilled nursing on call, day or night, whenever you need us.

100%

Care plans built and overseen by a registered nurse.

8 hrs

Minimum dedicated shift. Unhurried, present, fully attentive.

Going home shouldn’t mean doing it alone

  1. Led by a registered nurse, always

Every care plan is built and overseen by a registered nurse. A certified nurse may support once your loved one is stable, but a nurse is always accountable.

  2. Concierge coordination

We manage the moving parts, from physicians and medications to labs and appointments, so the family can simply be family again.

  3. Never alone, never rushed

Your loved one always has a nurse by their side, every hour of every shift, giving them complete and unhurried attention.

  4. Your physician, kept in the loop

We keep your physician and care team informed about the care plan and how your loved one is doing, so everyone stays on the same page.

How your care begins.

Start with a nurse consult
  1. 01

    Connect with a nurse

We listen, ask a few clinical questions, and confirm we’re the right fit.

  2. 02

    Assessment & matched plan

An RN reviews the diagnosis and goals, then matches the right certification.

  3. 03

    Care begins

Home, hospital, hotel, or on the road. Plus any equipment needed.

  4. 04

    Ongoing oversight

We watch for warning signs and keep your physicians in the loop.

Partners & nurses

Two more ways to work with us.

Whether you refer patients or care for them, we’ve made your next step simple.

For referral partners

When they leave your hands, they’re safe in ours

Anyone who cares for a patient at a skilled nursing facility can refer to us. Do any of these describe you?

  • Case manager or discharge planner
  • Director of nursing or nursing staff
  • Social worker or social services
  • You help coordinate a patient’s discharge

For nurses

If you’re a nurse

We’re building a team of registered nurses who believe care should be personal. Does this sound like you?

  • Registered Nurse (RN) or LVN
  • You want to care for one patient at a time
  • You’re looking for flexible, premium work

Questions families ask.

Anything we haven’t answered, a concierge RN will answer on the call.

A nurse and an older patient working through a tabletop exercise together

No. We are a private-pay care and don’t bill insurance directly, though we coordinate with any covered home health, hospice, or therapy you already have.

We match your case to an RN whose certification fits your diagnosis, and we keep your care team and plan as consistent as possible. For continuous coverage we may use a small matched care team rather than a single nurse, always briefed on your full plan.

Every Concierge Rx nurse is a registered nurse with hospital bedside experience, and we match your case to an RN whose specialty certification fits your diagnosis, such as CCRN, OCN, PCCN, ONC, or BMTCN.

Post-surgical recovery, oncology and transplant, cardiac and neurological recovery, and chronic or complex conditions, plus travel nursing and care coordination.

At home, at the hospital bedside, in a hotel, or while you travel.

No. For emergencies call 911. We provide planned and ongoing clinical support, not emergency response.

After a consult and assessment, we can often begin quickly, your concierge RN will confirm the timeline on the call.

Let’s talk about the care you need.
